3. Using the link on the registration-confirmation web page (under step 2), download the trade-in application form. Complete and submit the form to Patton or Patton-Inalp.
4. Within five days Patton will e-mail your claim number and shipping instructions for returning your trade-in equipment.
5. Patton will ship and bill you for your order at full value and issue a credit memo against your completed form.
6. Ship your old VoIP equipment to Patton or Inalp with the completed application and a copy of your order confirmation.
7. Once your returned goods are received, Patton will issue your Port BuyBack rebate by the same method you made payment (e.g. check, credit card, wire transfer, etc.) .

Patton VoIP IAD Port-Replacement Equipment Buy-Back Program
TERMS AND CONDITIONS

• Patton VOIP-PORT-REPLACEMENT Program is available to purchasers of new eligible Patton SmartNodes between 30 August and 31 March 2012 with the trade-in of an eligible VoIP product from any vendor. Trade-in VoIP units must be designated as “eligible” by Patton to receive a trade-in value. This credit may be applied only towards the purchase of Patton product. Patton Certified Channels may accept trade-in equipment only from end customers, not from brokers or other resellers.
• Claims/Orders must be received by 31 March 2012. 
• Customer warrants that it holds legal title to trade-in equipment, free of liens or security interests, and has the legal right to transfer that equipment to Patton.  Title to trade-in equipment transfers to Patton on the date and time trade-in equipment is received by Patton.
• The VoIP trade-in value cannot be combined with other offers.
• To receive full quoted trade-in value, trade-in VoIP unit must be returned with the ability to accept between 15 and 120 VoIP calls and must present at least 1 T1/E1/PRI interface port.
• Payment to the end-customer returnee, or credit to the reseller will be effected within approximately 30 days of receipt of an approved claim and all required materials, including trade-in VoIP. This program is a one-for-one trade. Currency and exchange rates are at Patton's discretion.
• All units must be shipped to Patton facilities in Gaithersburg Maryland or Berne, Switzerland. Shipping and delivery of trade-in equipment must occur as set forth in these terms and conditions and is the sole responsibility of the sender.
• If trade-in equipment is received in excess of 30 days after the expiration date of this program, the claim will be cancelled. In no event will Patton accept trade-in equipment after 30 April 2012. Equipment received after that date will be returned to the sender at the sender’s expense.
• The delivery of trade-in equipment to Patton is final. No trade-in equipment or accessories can be returned to the customer or reseller once Patton has received it. Upon receipt, Patton will make payment to the reseller or customer in approximately 30 days.
• All incomplete claims will be automatically rejected, and must be completed and resubmitted in order to be processed. An incomplete claim is defined as missing any requested information. 
  Patton reserves the right to cancel or modify this program, and other Patton promotional programs, at Patton’s discretion.
• Customer agrees to indemnify and hold Patton harmless from any and all causes of action, costs, loss, attorney fees or the like associated with the traded product.
• Patton express warranty to purchaser constitutes Patton sole liability and the purchaser’s sole remedy with respect to the products, and is in lieu of all other warranties, liabilities and remedies.
• Rebates will be made payable to companies only, no rebates will be paid to individuals.
• No substitutions allowed. Available to all customers and channels following standard Patton price lists. Deviated prices not eligible and are disqualified. Patton's commercial terms, conditions and credit policies apply. Not valid in conjunction with any other offer or discount. Patton reserves the right to modify terms without notice. Void where prohibited.
